A Study on the Animal Model for Dementia Induced by Scopolamine

Abstract

This study aimed to determine dose of scopolamine to induce cognitive and memory dysfunction in rat. Six-week-old male rats (n=20) were randomly divided four groups. Scopolamine bromide (0, 0.5, 1.0 and 2.0 mg/kg) was intraperitoneally injected for 7 days. From 4-7 days after injection, the rats were performed memory test by water maze and passive avoidance test. On the 8th day, the rats were sacrificed, then acetylcholinesterase (AChE) activity, cholinergic proteins and inflammatory mediators were analyzed from their brain tissue. In the memory test and AChE activity, the group of scopolamine treatment (0.5 mg/kg) showed no significant difference compared to the control group (0 mg/kg). But, they showed significant differences compared to the control group in expression of choline transporter, choline acetyltransferase, and vesicular acetylcholine transporter protein and nuclear factor kappa B, tumor necrosis factor-α, interLeukin-6, inducible nitric oxide synthase, and cyclooxygenase2 mRNA. Also, the groups of scopolamine treatment (1.0 and 2.0 mg/kg) showed significant differences compared to the control group in memory test, AChE activity, expression of cholinergic proteins and inflammatory mediators mRNA. Therefore, it was suggested that 1.0 mg/kg of scopolamine is adequate dose to induce cognitive and memory dysfunction in rats.

Introduction

전 세계적으로 치매 환자는 약 5,500만 명으로 추정되고 있으며, 거의 모든 나라에서 인구 중 고령자 비율이 계속 증가하고 있기 때문에, 치매 환자의 수는 2030년에는 7,800만 명, 2050년에는 약 1억 3,900만 명으로 증가할 것으로 예상된다. 우리나라는 전 세계에서 가장 빠른 속도로 고령화가 진행되고 있으며, 통계청 인구추계에 따르면 2045년 전체 인구 중 46.5%가 65세 이상의 노인이 될 것으로 보고하고 있다(Statistics Korea, 2020). 이와 같이 노인 인구가 늘어나면서 노인성 질환이 국가적 문제로 대두되고 있고, 노인성 치매 환자의 수 역시 급증하고 있는 추세로, 치매 환자의 수는 17년을 주기로 두배 씩 늘어날 예정이며, 2024년에는 100만 명, 2039년에는 200만 명, 2050년에는 300만 명을 넘어설 것으로 예측되고 있다(Arvanitakis et al., 2019).

치매란 뇌의 위축, 뇌혈관 장애, 뇌세포 소실 등의 다양한 원인으로 뇌기능이 손상되어 인지기능저하 및 기억 손상으로 정상적인 일상생활을 할 수 없는 뇌노화 증후군으로 알려져 있으며(Terry & Davies, 1980), 현재까지 뚜렷한 치매의 원인과 발병기전이 밝혀지지 않았을 뿐만 아니라, 확실한 치료제가 발명되지 않아 현재 많은 후보 물질들이 유효성과 안전성 검정을 하고 있는 실정이다(Cummings et al., 2021; Huang et al., 2020).

특히, 유효성을 위한 동물실험에서 많은 치매 동물모델이 개발되어 이용되고 있다. 대표적인 치매 동물 모델로는 자연노화 모델, 형질전환 모델, 화학물질 이용한 모델 등이 있다(Drummond & Wisniewski, 2017; Esquerda-Canals et al., 2017). 자연노화 모델과 형질전환 모델의 경우, 치매 발생 기전이 사람에서의 발생기전과 유사하여 많이 사용되고 있지만, 발생되기 까지의 시간이 많이 필요하다는 점과 사육관리의 문제 및 고가의 동물 가격이 단점으로 작용하고 있다(Park et al., 2013; Park et al., 2020). 화학물질로는 아밀로이드 베타, 콜린성 독소 및 스코폴아민 등을 사용한 동물 모델 등이 있으며, 이들은 경제적으로 싸다는 장점이 있지만, 기억력 결핍이 유지되는 기간이 짧다 라는 단점과 때때로 고도의 전문적인 기술이 필요하다는 단점이 있어, 치매 치료제 개발 시 약리기전에 맞게 여러 모델을 병용하여 평가하고 있다(Gilles & Ertle, 2000; Nakai et al., 2021; Park et al., 2012; Zeiss, 2020).

여러 치매 유발 동물 모델 중 스코폴아민은 콜린성 기억력감퇴 동물모델에 주로 이용되고 있는 약물로써 신경전달 물질인 아세틸콜린과 무스카린성 콜린 수용체 간의 결합을 저해하는 한편, 아세틸콜린에스테라제의 활성을 증가시켜 아세틸콜린의 활성을 감소시킴으로써 신경전달 경로를 차단하는 것으로 알려져 있다(Chen & Yeong, 2020; More et al., 2016; Tang, 2019). 스코폴아민은 단기간에 기억 손상을 유발시키기 때문에, 동물실험에서 기억력 결핍 및 인지 능력 저하에 대한 연구 분야에 널리 사용되고 있지만 기억력 결핍 유지 기간이 짧아 스코폴아민의 사용 용량의 범위가 다양하며 체계화되어 있지 않다(Klinkenberg & Blokland, 2010).

따라서, 본 연구에서는 스코폴아민의 현재 연구에서 많이 사용되고 있는 용량(0, 0.5, 1.0 및 2.0 mg/kg)을 기준으로 기억력 검사를 위한 행동 검사 및 콜린성 신경계의 활성 및 염증 반응 관련 지표를 분석하여 기억력 결핍을 위한 최적의 용량을 찾아 향후 스코폴아민을 이용한 치매 동물모델 연구에 기초 정보를 제공하고자 하였다.

Materials and Methods

Materials

Animals

본 실험의 동물실험은 6주령의 수컷 Sprague Dawley Rat (n=5/군)을 대한바이오링크 (Eumseong, Korea)에서 구입하여서 진행하였다. Rat는 온도 22 ± 3°C, 상대습도 50 ± 10%, 조명 시간 12시간(07:00~19:00)로 설정하여 사육관리 하였다. 사료는 실험 동물용 고형사료를 대한바이오링크(Eumseong, Korea)에서 구입하여 사용하였으며, 사료 및 음수는 자유 섭취시켰다. 본 연구는 충북대학교 동물실험 윤리위원회에서 승인받아 진행하였다(CBNUA-1594-21-01).

Treatment of Scopolamine

본 실험에 사용한 Scopolamine hybromide (Sigma-Aldrich, St. Louis, MO, USA)는 in vivo 실험에 사용하고자 0, 0.5, 1.0, 및 2.0 mg/kg의 용량으로 PBS에 녹여 조제하였다. 각 군당 5마리씩 총 4군으로 나누어 실험하였으며 대조군에는 PBS를 투여하였고 실험군에는 용량별로 조제한 스코폴아민 용액을 5 mL/kg의 용량으로 7일간 1일 1회 복강 투여하였다. 복강투여 하는 기간의 4~7일은 스코폴아민 용액을 복강투여하고 30분 후에 행동 검사를 실시하였다. Methods

Passive Avoidance Test

설치류의 working memory ability를 측정하기 위해 shuttle box (width (610 mm) x depth (300 mm) x height (400 mm))를 이용해 수동 회피 반응(passive avoidance)을 평가하였다. Box는 램프와 감전을 위한 강철 격자 바닥이 장착된 밝은 방과 어두운 방으로 나뉜 두 개의 방으로 구성된다. 실험에서는 Rat가 문을 통해 밝은 방에서 어두운 칸으로 들어갔을 때 전기 충격(3초, 0.3 mA)을 받았다(Hermans et al., 1992). 대기 시간은 조명실의 조명이 켜지고 나서 밝은 방으로 들어가는 시간으로 설정하였으며, 300초가 넘어가면 기억력이 회복되었다고 판단하여 실험을 종료하였다. 수동 회피 실험은 스코폴아민 투여 4일 ~ 7일까지 1회/일, 4일간 측정하였다.

Morris Water Maze Test

공간 기억의 평가를 위해 Morris 수중 미로 실험을 진행하였다. 수중 미로 실험은 물이 27 cm의 깊이로 채워진 직경 180 cm의 수조에서 진행하였으며 물의 온도는 22 ± 3°C로 유지되었다. 수조는 가상의 4개의 사분면으로 나누어 한쪽 사분면 중앙에 escape platform을 수면 2 cm 깊이 아래에 위치시켰다. 이후 먹물을 풀어 platform이 보이지 않도록 하였고 Rat는 미로 바깥에 놓인 여러 단서에 근거하여 platform을 찾는 훈련을 받았다. 단서의 위치는 실험 내내 일정하게 유지되었다. 사분면 중 platform이 없는 3개의 사분면 중 한 지점에서 출발하게 하여 plat form을 찾기까지 걸린 시간을 측정하였다(Morris, 1984). 수동 미로 실험은 스코폴아민 투여 4일 ~ 7일까지 1회/일, 4일간 측정하였다.

Acetylcholinesterase (AChE) Activity in Brain

뇌조직은 protease inhibitor 칵테일 (Sigma-Aldrich, St. Louis, MO, USA)이 함유된 9배의 PBS를 첨가하여 균질화 하였다. 13,500 rpm에서 4°C에서 6분간 원심분리 후 0.1 M phosphate buffer (pH 8.0) 2.6 mL, 10 mM Ellman’s 용액 (10 mM 5,5'-dithio-bis-2-nitrobenzoic acid (Sigma-Aldrich, St. Louis, MO, USA), 15 mM sodium bicarbonate (Sigma-Aldrich, St. Louis, MO, USA) 100 μL, 뇌 균질액 0.2mL를 cuvette에 넣은 후 75 mM acetylcholine iodide (Sigma-Aldrich St. Louis, MO, USA) 20 μL를 첨가하여 405 nm에서 1분간격으로 5분간 분광광도기(SpectraMax M2, Molecular Devices, San Jose, CA, USA)를 이용하여 측정하였다.

Real-time PCR Analysis

뇌 조직의 total RNA는 Trizol 시약 (Invitrogen, CA, USA)을 사용하여 제조업체의 프로토콜에 따라 추출하였다. Total RNA 1 μg은 WizScriptTM RT Master (Wizbio, Seongnam, Korea)를 이용하여 cDNA를 합성하기 위한 역전사에 활용되었고, 생성된 cDNA는 PCR을 위한 template로 이용하였다. Real time PCR은 Brilliant III Ultra-Fast SYBR® Green QPCR Master Mix (Agilent Technologies, Santal Clara, CA, USA)를 사용하였으며, 반응은 60℃, 40 cycle로 AriaMx Real-Time PCR system (Agilent Technologies)에서 수행하였다. 내부 기준 물질로는 GAPDH를 이용하였고, comparative Ct method를 사용하여 2회 반복하여 데이터를 분석하였다(Yon et al., 2018). PCR에 사용된 primer의 sequence는 Table 1에 표기하였다

Western Blot Analysis

뇌조직은 proteinase inhibitor (Sigma-Aldrich, St. Louis, MO, USA) 및 phosphatase inhibitor (Sigma-Aldrich, St. Louis, MO, USA)을 함유한 RIPA cell lysis buffer (Sigma-Aldrich, St. Louis, MO, USA)을 이용하여 균질화 하였다. 단백질은 균질액을 4℃, 15,000 rpm에서 15분간 원심 분리하여 추출하였으며 BCA protein assay kit (Pierce, Rockford, IL, USA)를 이용하여 정량 하였다. 단백질은 10% SDS가 함유된 0.5 M Tris-HCl buffer (pH 6.8)에서 95℃에서 5분간 가열하여 변성시킨 후 SDS-PAGE gel에서 전기 영동 하여 분리시킨 후, 20% methanol, 1% SDS 및 192 mM glycine이 함유된 25 mM Tris buffer를 사용하여 PVDF membrane으로 이동시켰다. TBS-Tween (TBS-T 20mM Tris, 137mM NaCl, 0.1% Tween 20, pH 7.6) 용액을 이용하여 5% skim milk를 제조하여 1시간 blocking 한 후 Table 2에 있는 항체를 이용하여 4℃에서 overnight하여 반응시켰다. TBS-T로 세척한 후, membrane에 Horseradish peroxidase-conjugated anti-rabbit 2차 항체를 이용하여 실온에서 2시간 반응시켰다. Membrane은 ECL solution (Pierce, Rockford, IL, USA)을 이용하여 현상하였다. Actin을 내부 기준 물질로 하여 ImageJ (NIH, Bethesda, Maryland, USA)를 이용하여 정량 하였다.

Statistical Analysis

그룹 간의 통계 비교는 정규성 및 분산의 동질성을 확인하고, 일원 분산분석을 수행한 후 Tukey의 다중 비교 테스트를 통해 사후 검증하였다. 모든 분석은 윈도우용 소프트웨어인 Statistical Package for Social Sciences version 12.0 (SPSS Inc, Chicago, IL, USA)을 통해 수행하였다. 데이터는 평균의 평균 ± 표준 오차로 표시하였고, P-값 <0.05 인 경우 통계적으로 유의한 것으로 간주하였다.

Results

Memory Deficit Induced by Scopolamine

Passive Avoidance Test

해마의 조건화 된 기억과 변연계와 연관 있는 기억력을 측정하는 방법으로 알려진 수동회피실험에서 정상군인 대조군은 어두운 방의 전기 충격을 기억해 훈련이 진행될수록 점차 밝은 방에 머무는 시간이 길어졌으나 이에 비해 스코폴아민 1.0 및 2.0 mg/kg 투여한 경우 기억 소실로 전기쇼크에도 불구하고 검은 방으로 넘어가는 시간이 짧다는 걸 확인할 수 있다. 스코폴아민 0.5 mg/kg 투여한 실험군은 둘째날에서 셋째날로 갈수록 대조군과 유의미한 차이는 보였지만 평균의 차이는 감소하는 것을 확인할 수 있었으며, 4일차에는 대조군과 유의미한 차이를 보여주지 못하였다(Fig. 2A).

Morris Water Maze Test

스코폴아민으로 유도된 기억력 손상에 대한 정도를 확인하고자 공간지각 학습능력을 측정할 수 있는 수중 미로 실험을 수행하였다. 실험동물이 주변의 단서를 파악해 보이지 않는 플랫폼을 찾는데 걸리는 평균 탈출시간을 비교해본 결과, 대준군은 훈련 첫째날에 비해 훈련 기간이 길어질수록 플랫폼을 찾는 시간이 현저하게 감소하여 장기 기억이 형성되었다고 할 수 있으나, 스코폴아민 1.0 및 2.0 mg/kg 투여한 실험군은 훈련 기간과 상관없이 탈출 소요 시간의 변화가 거의 없었으며 대조군과 유의한 차이를 보이지 못했다. 따라서, 플랫폼을 찾지 못하고, 기억 손상이 잘 유도되었다고 확인할 수 있었다. 이때 스코폴아민을 0.5 mg/kg 투여한 실험군은 대조군과 비교해 볼 때, 스코폴아민에 의한 기억손상으로 탈출시간이 2배정도 지연되긴 하였으나 첫째날부터 넷째날까지 훈련에 따라 점차 탈출시간이 감소해 대조군과 유사한 그래프 양상을 보이며, 3회차부터는 유의미한 차이를 보이지 못하므로 1.0 및 2.0 mg/kg 용량의 스코폴아민 투여군처럼 유의미한 기억 손상이 이뤄졌다고 보기 어렵다(Fig. 2B). 이에 행동실험 모두에서 스코폴아민 1.0 및 2.0 mg/kg 투여군에서만 유의미한 차이를 보였다.

AChE Activity in Brain Tissue

스코폴아민 용량에 따른 AChE의 활성을 분석하기 위해 7일간 스코폴아민을 투여하고 부검 후 뇌조직을 이용하여 AChE의 활성을 측정하였다. 스코폴아민 1.0 및 2.0 mg/kg 투여한 실험군은 대조군과 유의미한 차이를 보이며 AChE 활성이 증가하였다. 하지만 동물행동실험과 마찬가지로 스코폴아민을 0.5 mg/kg 투여한 실험군은 대조군과 유의미한 차이가 없었다(Fig. 3).

Expression of CHT, ChAT, and VAChT in Brain Tissue

스코폴아민의 투여가 콜린성 신경계주는 영향을 분석하기 위해, 아세틸콜린 합성과 관련된 단백질의 발현량을 western blot을 통해 비교하였다. Choline transporter (CHT), choline acetyltrasferse (ChAT), 소포아세틸콜린수용체(vesicular acetylcholine transporter, VAChT)의 발현량을 비교해 본 결과, 스코폴아민 0.5, 1.0 및 2.0 mg/kg의 투여로 인해 대조군과 유의미한 차이를 보이면서 발현이 감소했다(Fig. 4).

Inflammatory Mediators in Brain Tissue after Treatment of Scopolamine

스코폴아민 투여에 따른 뇌조직 내의 염증 반응 정도를 비교하기 위해, 부검 후 뇌조직으로부터 mRNA를 추출한 후 cDNA를 합성하여 real-time PC을 이용하여 염증반응 발생에 중요한 신호 전달물질인 nuclear factor kappa B (NF-κB), 염증 매개성 사이토카인인 tumor necrosis factor-α (TNF-α) 및 interLeukin-6 (IL-6), 그리고, 염증매개 효소인 inducible nitric oxide synthase (iNOS), 및 cyclooxygenase2 (COX2)의 발현량을 확인하였다. 발현량의 차이는 있었으나, 모든 유전자에서 스코폴아민 0.5, 1.0, 및 2.0 mg/kg의 투여군 모두가 대조군과 유의미한 차이를 보이면서 발현이 증가했다(Fig. 5).

Discussions

본 실험은 스코폴아민으로 유도한 기억 손상 동물모델 효능 실험에서 효과적인 투여 용량을 설정하기 위해 행동검사, 콜린성 신경계의 활성, 및 염증지표를 분석하는 실험이었다.

치매는 원인이 정확하게 알려져 있지는 않으나 현재는 두 가지 이론이 받아들여지고 있다. 하나는 베타 아밀로이드 단백질의 축적으로 인한 뇌세포의 사멸이며, 다른 하나는 타우 단백질의 과산화에 의한 뇌세포의 손상이다(Ashrafian et al., 2021). 축적된 베타 아밀로이드 단백질은 콜린성 신경을 파괴하고 기억력 손상을 유발하며, 타우 단백질의 과산화 역시 시냅스 전달에 문제를 야기하여 기억력 손상을 유발한다(Gao et al., 2018; Pinheiro & Faustino, 2019) . 최근 개발되고 있는 바이오 신약들은 이러한 단백질들을 제거하는 것을 목적으로 하고 있지만, 아직까지 효과적인 치료제는 개발되지 않고 있는 실정이다.

이로 인해 현재 개발되고 있는 약들은 기억력, 학습능력 및 인지 능력 저하 같은 증상을 완화시키는 것을 목적으로 하고 있으며 대표적은 donepezil등과 같은 약물이 이러한 역할을 하며, 국내에서도 천연물 등을 이용한 많은 실험이 진행되고 있으며, 이 때 많이 사용되는 동물 모델이 스코폴아민 모델이다(Ferreira-Vieira et al., 2016; Guo et al., 2021; Oh et al., 2021).

스코폴아민 약물의 특성상 0.5 ~ 20.0 mg/kg까지 다양한 농도로 사용되고 있지만(Bejar et al., 1999; Jafarian et al., 2019), 본 실험에서는 약물의 효율적인 사용을 위해, 0.5, 1.0, 및 2.0 mg/kg의 용량으로 실험을 진행하였다. 먼저 행동검사를 하였을 때, 0.5 mg/kg을 투여한 동물에서는 수중 미로 실험과 수동 회피 반응에서 모두 처음에는 기억 습득 능력이 저조하였으나 차차 기억력을 회복하는 모습을 보여주었다. 반면 1.0 및 2.0 mg/kg에서는 기억 습득 능력이 생성되지 않은 것을 확인할 수 있다. 다른 연구에서, 0.5 mg/kg의 용량을 사용한 실험에서는 본 실험과 비교했을 때 비교적 장기간 약물을 투여하여 행동검사를 실시하여 기억력감퇴를 유발하거나, 본 실험과 같이 단기간에 회복되는 경향을 보인다(Gattu et al., 1997).

마찬가지로, AChE의 활성 검사에서 0.5 mg/kg의 용량에서는 AChE의 활성이 대조군과 비교했을 때 유의미한 차이를 보이지 않았으며, 1.0 및 2.0 mg/kg 용량에서는 농도의존적으로 증가한 것을 확인할 수 있다. AChE는 시냅스에서 작용하여 아세틸콜린을 분해하는 효소이다. AChE가 활성화되면 아세틸콜린의 작용시간이 감소하기 때문에 기억력 결핍을 유도하는 효과를 보이게 된다. 현재 치료제로 쓰고 있는 donepezil이나 tacrine 같은 물질들은 이 AChE를 억제하여 아세틸콜린의 농도를 일시적으로 상승시켜 기억력을 회복시키는 용도로 사용하고 있으며, 많은 물질들이 이 AChE의 억제를 목적으로 개발되고 있다(Marucci et al., 2021; Shin et al., 2018). 하지만, 0.5 mg/kg의 용량에서는 AChE의 활성을 충분히 상승시키지 못하기 때문에 AChE의 억제를 목적으로는 좋은 용량이 되지 못할 것으로 보인다.

또한, 알츠하이머 환자에게서 나타나는 인지 기능 장애는 콜린성 신경의 손상에 의한 것이라는 여러 연구 결과가 발표되었다(Gais & Born, 2004; Rogers & Kesner, 2003). 아세틸콜린은 기억력 형성과 밀접한 관련이 있는 신경전달물질이며, 아세틸콜린 농도의 감소는 인지능력 저하와 관련이 있다고 보고되었다(Perry, 1988). 따라서, 치매를 유발한 동물에서는 아세틸콜린의 합성과 관련이 있는 단백질의 발현이 감소되어 있으며, 이러한 특징을 이용하여 콜린성 독소를 치매동물 제작에 이용하기도 한다(Park et al., 2012) . 본 실험에서도, 스코폴아민 처리 후 아세틸콜린의 합성과 관련된 CHT, ChAT, VAChT의 발현을 확인하였을 때, 대조군에 비해 유의미하게 감소하는 것을 확인하였다. 따라서, 아세틸콜린 합성 전구물질을 치료제로 사용하는 실험에 동물모델로 사용할 수 있을 것으로 예측된다.

콜린성 신경계의 손상 이외에도, 베타-아밀로이드 단백질의 축적은 염증반응을 증가시켜 신경세포를 손상시켜 세포사멸을 유도하는 것으로 알려져 있다(Kinney et al., 2018). 염증성 사이토카인의 증가는 또한 베타-아밀로이드 생성과 관련이 있는 단백질의 발현을 증가시키는 것으로 보고되었다(Alasmari et al., 2018; Sommer et al., 2017). 이러한 사실을 근거로 몇몇 연구에서는 염증 유발물질인 LPS를 뇌내로 투여하여 기억력 결핍을 제작한 후 실험을 진행하였다(Kitazawa et al., 2005; Zhao et al., 2019). 본 실험에서도 스코폴아민을 처리하면 모든 용량에서 염증 발생 신호전달물질인 NF-kB, 염증 매개성 사이토카인(TNF-α, IL-6) 및 염증 매개효소(iNOS, COX)의 발현이 대조군과 비교했을 때 유의미하게 증가하였다. 이러한 결과는 스코폴아민을 처리하면 TNF-α, IL-6, IL-1β와 같은 염증 사이토카인의 방출을 억제해주는 아세틸콜린이 감소하게 됨으로써 염증반응이 증가한다는 보고와 일치했다(Sun et al., 2013).

이상의 결과를 종합하면, 스코폴아민을 이용한 치매 동물모델 제작은 유발 기간에 따라 용량의 영향을 받을 수 있지만, 약 7일간 투여하는 실험조건에서는 0.5 mg/kg에서는 효과가 없었으며, 1.0 및 2.0 mg/kg에서 효과가 나타났다. 특히 0.5 mg/kg에서는 CHT, ChAT, VAChT 및 염증지표에서는 그 효과가 나타났지만, AChE 활성 검사와 행동검사에서는 그 효과가 나타나지 않아 치매 유도 용량으로는 적합하지 않았으며, 1.0 및 2.0 mg/kg에서는 모든 항목에서 대조군과 유의미한 차이를 보였다. 스코폴아민을 이용한 치매 동물 모델은 행동검사 및 AChE 활성 검사, 아세틸콜린 합성 관련 단백질 및 염증 지표에서 대조군과 차이를 보였다. 특히 1.0 과 2.0 mg/kg에서는 용량 간에는 유의미한 차이가 보이지 않는 것으로 보아 1.0 mg/kg을 최적의 용량으로 제시하고자 한다.
